5. SSS ID Number (Employer/Self-Employed)
6. Company Profile & Product Line, Composition of the Corporation/Management Staff
7. PhilGEPS Registration/Organization Number (Red Membership) or Certificate of PhilGEPS Registration (Platinum Membership)
8. *Notarized Omnibus Sworn Statement (template may be requested by emailing firstname.lastname@example.org)
*(Notarized Omnibus Sworn Statement may be submitted within the submission of offer / evaluation of offer / or before issuance of Notice of Award.)
*For items that are on process of updating/renewal, kindly state on a letter that submission shall be made immediately upon receipt/issuance of the documents.
