REQUEST FOR QUOTATION

The Department of Environment and Natural Resources (DENR), Region-9, Regional Office, through its Regional Bids and Awards Committee (RBAC), will undertake a Small Value Procurement for the -œSupply and Delivery of Fifty (50) bundles Polyethylebe bag (4x6), etc., in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184 .

Approved Budget for the Contract: Php 50, 000.00

Location: DENR-9, Regional Office, Barangay Balintawak, Pagadian City

Delivery Term: Within fifteen (15) calendar days from the receipt of the Notice to Proceed

Interested Suppliers are required to submit their valid and current Mayor-s Permit, Income/Business Tax Return (for ABCs above Php500k), PhilGEPS Registration Number, Omnibus Sworn Statement and Authority of Signatory, upon submission of quotation. PhilGEPS Platinum Membership maybe submitted in lieu of the Mayor-s/Business Permit and the PhilGEPS Registration Number. Procurement of expendable supplies, a warranty shall be required from the contract awardee for a minimum period of three (3) months, after acceptance by the office of the delivered supplies, per Section 62.1 of the 2016 Revised IRR. The obligation for the warranty shall be covered by either retention money in an amount equivalent to at least one percent (1%) of every progress payment, or a special bank guarantee equivalent to at least one percent (1%) of the total contract price.
